Dr. Sameh interrupts his annual leave and asks his manager for permission to oversee Noah's critical case. His manager forbids him considering the risk involved. Noah fights an inmate and is isolated in a room. Then a nurse is found hung in a room with blooded question marks printed on the walls.

Before Noah was in the hospital, he lived in a villa with his family and did drugs with his brother. Sameh tries to work on Noah's case after the nurse's suicide. He contacts his deceased colleague's wife for information about the case, and receives a surprising phone call.

Sameh receives a call from someone claiming to be Tarek, so he contacts Tarek's wife for details about his death. Dr. Mahmoud agrees to Sameh working on Noah's case. Sameh goes to Noah at the hospital, where Noah has a confession.

Noah tells Sameh about Tarek's suicide. Sameh receives mysterious phone calls, and Hemat pays Dr. Rabei a visit. Noah decides to disclose everything to Sameh.

Rihab enters the scene, and develops a relationship with Noah. Qandil spies on Noah. Noah confesses to Sameh that he killed Tarek. Jasser reflects back on a previous relationship. Noah devises a plan with Shahin's help.

Noah finds out about his brother selling drugs and they get into a conflict. Sameh tries to get closer to Noah. Noah's father indirectly refuse to visit him in the hospital. Noah threatens Sameh in order to get out of the hospital.

Qandil is the reason for the struggle between Noah and Jasser, which has resulted in an unintended gunshot. Noah leaves the gun on Sameh's desk and asks Dr. Mahmoud to keep it in a safe place. Sameh sees Noah and drops to the floor with a bloody head. Rehab's aunt mourns Rehab.

Memories for Al-Shawwaf family surface. Reem, Sameh's fiancée, is friends with Noah's cousin, Marwa, and has feelings for Noah. Noah's father wanted to see Jasser, in his final days. Jasser was heartbroken over the death of his fiancée, Alaa'. Sameh recovers from his head wound.

The struggle between Noah and Jasser results in Noah getting wounded. Sameh investigates in the hospital and finds out that there was a conflict between Noah and Yacoub. Sameh goes to Sheikh Mansour and finds out about him meeting with Tarek. Sameh gets a threatening note.

Noah and Jasser reconcile. Reem breaks up with Sameh when she sees his with Hemmat. Hemmat is having an affair with Dr. Mahmoud. Sameh gets conflicting information from Marwa Al-Shawwaf. Dr. Mahmoud, Hemmat and Sheikh Mansour all conspire against Sameh. Sameh's lurker is revealed.

Rehab's cousin is in a relationship with Qandil. Noah is angry with Marwa because of her mistreatment towards Qandil. Noah confesses to Sheikh Mansour that he is devastated because of the situation with Jasser. Reem gets in trouble while on a video call with Sameh.

Jasser spends time with Rehab and imagines that she is Alaa', his deceased fiancée. Fouad asks Noah and Marwa to get married. Sameh looks for Reem, knowing that she has been kidnapped. Sameh discovers from Tarek's notes that Hemmat was cheating on him with Dr. Mahmoud.

Noah and Marwa devise a plan. Sabah makes a move on Noah in his car, and he violently rejects her. Noah confesses to Rehab that he loves her. Sameh meets with Reem's sister, who treats him badly. Noah recalls his relationship with Reem, Sameh's fiancée. Shawqi has dark plans for Rabei.

Noah confesses his love to Rehab. Fouad tells Jasser that his father's dying wish was for him to marry Marwa. Rabei discovers that Shawki has betrayed him. Sameh calls Jasser to get him to go back to Egypt. Sameh discovers that Hemmat has been missing for six months.

Qandil and Marwa confess their feelings to one another. Rehab goes out with Noah and returns late, and her aunt threatens to call Noah and tell him the truth. Sameh discovers Reem's relationship with Noah. Sameh calls Dr. Mahmoud in an attempt to receive some information. Jasser returns to Egypt.

Jasser decides that he will marry Marwa and informs Noah and Rehab about his decision. Jasser tells Sameh that Noah is a murderer and a drug dealer. Marwa tries to reach Reem, and Yacub tries to discourage her from getting involved. Sameh meets with Tarek.

Jasser tries to talk to Qandil but he refuses. Marwa devises a plan to get Sabah to marry Yacub in return for money. Shahira, Fouad Al-Shawwaf's wife, is murdered, and Aesha and Qandil both witness the incident. Alaa's murderer is revealed, and more murders happen.

Qandil reaches an agreement with Rehab against Al-Shawwaf family. Jasser learns the truth about Marwa and Noah, and makes a deal with her in exchange for his inheritance. Sameh learns that Alaa' cheated on Jasser with Anas, Rehab's fiancé. Noah tells him that Rehab is alive. Haitham is missing.

Yacub wants Jihan to tell Qandil about his marriage to Sabah. Sameh finds himself in a confusing and bloody situation with Jasser. Sameh goes into Qandil's room and tells Noah about Jasser. Noah decides to escape from the hospital with Qandil and Sameh, while knowing the identity of the killer.

Jasser tells Marwa that his inheritance is her dowry. Tarek goes to Rehab's aunt's home. Sameh and Noah escape to Jihan's home. Aziza tells her friend about her feelings for Noah. Sameh and Noah meet with Sheikh Mansour.

Noah discovers Qandil's true colors. Tarek tries to persuade Rehab to seek treatment. Yacub asks Sabah to tell Noah about their marriage. Ashmawi refuses to work with Noah. The police tell Fouad about the murder of Jasser and Noah's disappearance. Hemmat confronts Jihan.

Tarek attempts to persuade Rehab to seek therapy. Noah threatens to kick Sabah and her family out of the villa. Jasser and Marwa get married. Sameh asks Noah to be careful from Sheikh Mansour. Sameh comes across Tarek's notes and makes a strange discovery.

Qandil leaves the house in anger after Marwa's marriage. Rehab tells Noah that Jasser harasses her verbally. Sameh captures the homeless person who tells him that he is hired by Noah. Aziza tells Noah about a dream she had and asks him to stay away from Sameh.

Marwa tells Jasser that Noah harasses her verbally. Rehab meets with Jasser who tells her that he knows that she is tricking Noah. Shawqi and Aziza decide to help Sameh. Reem, Rehab, Iman and Marwa go to Jihan's house and hide when Noah comes.

Noah discovers Rehab's game, and gets into a confrontation with Jasser. Marwa is ready to leave Jasser and marry Qandil on the condition that Noah is killed. Hemmat tells Qandil that his sister is safe as agreed. Noah decides to go to Rehab's grave and is surprised with something at the graveyard.

Rehab insults her ex. Qandil comes to Noah and Noah asks for his help. Noah discovers that Rehab is alive. Noah decides to help Ashmawi marry Aziza. Rehab goes to Sheikh Mansour's house where Noah sees her, and she is joined by Jasser.

Qandil asks Noah to come with him to propose to Amira. Qandil is being treated by Dr. Tarek and meets Rehab in the clinic. Noah threatens Yacub to divorce Sabah. Yacub comes back from abroad, unaware that someone intends to kill him.

Yacub tells Noah that Fouad wants to get rid of him and Jasser. Noran advises Rehab to quit the game she's playing on Noah. Noah discovers that Marwa knows Fouad's intentions. Rehab learns about Yacub's death. Sameh learns that Rehab was having an affair with Jasser, and Reem was Qandil's wife.

Anas tells Jasser that Rehab murdered Alaa'. Rehab admits everything to Noah and he decides to shoot at her. Rehab confronts Sameh. Sameh meets Reem, who insults him and asks him to leave. Sheikh Mansour has sinister plans for Noah.

Sabah is married to Yacub and Qandil is married to Reem. Sameh, Rehab's brother, takes his sister's revenge from Noah in a different way. Qandil wants to hurt Al-Shawwaf family. Jasser and Rehab die in a car accident. Sameh gives an order to release Noah from the mental institution.
